Community Based Support
We provide community-based supports for people with disabilities, where each individual is encouraged to pursue their goals through accessing skills-based learning, recreation, and social activities. Working together with you and your family or support network, Community Carers Hub will work to create a personalized plan around your goals and aspirations identifying the programs and supports that will best assist you to achieve these.
